According to Raja Riku, Mansherry has special powers, which is the only reason why Doflamingo hasn't killed him.
It is then revealed that he has the ability of Devil Fruit which is directly able to heal the injury of living things in a way like through his tears. This is evident when he uses it for himself, provided his tears fall to the recipient. The greatest strength of this fruit is that Doflamingo thinks of utilizing recovery from inanimate objects, the recovery in his mind is SMILE, whatever he will do, even though it will shorten the life of Mansherry when using it.

During his detention, Doflamingomenipu's subordinates from the Dwarves who were slaves said that Mansherry was sick and needed a SMILE to cure him.
After the enslaved Dwarves rebelled, they discovered that Mansherry was not at all in the factory. He reveals he was imprisoned in the royal palace trapped in a small cell alone, he cries and begs Leo to come to his rescue.
After Sugar fainted, Mansherry was forced to use healing powers to heal him again.
Jora then tried to force him to heal the defeated Donquixote Pirates. Mansherry refuses, realizing that they are enemies and has no intention of helping them. After Jora learns that Mansherry's tears are also capable of healing, Jora tries to make her shed tears with her strength. However, before Jorabisa revived his injured colleagues, Leo and Kabu arrived just in time. Leo defeats Jora and saves Mansherry by capturing him with excitement.
Mansherry then gently asks Leo to carry him, saying that his leg hurts. Leo rejects his request and says that he shows his selfish side. Leo then says that he can cure his own feet. Mansherry grumbles stating that he will not run away and tells Leo that he never asked him to save him. When Leo is angry at Mansherry's mood swings, Kabu hits Leo's head and tells him to obey the Princess's orders. Leo then did it and they left the room where Mansherry was before.
